Templates and decision support prompts

These tools are aimed at clinicians (Nurses, GPs, ANPs, Pharmacists, Paramedics) delivering LTC reviews. Unlike many templates these are not full of tick boxes for data entry – these templates are all about foregrounding relevant information in one place to make reviews quicker and easier. The templates highlight relevant guidance in a static way in some cases (reference material) and at other times in a dynamic way (by using IT to interpret the guidelines and apply them to the specific patient).

Traffic Light System

The screenshot here shows a section of our heart failure template for a patient who is not optimised in terms of their heart failure care. We have used a traffic light system to highlight these care opportunities. There are other tabs in the template that link to guidance on why these areas are highlighted and how to action them.

THS also uses pop-up decision support prompts where there is something really important to action or something that is easy to action in a few clicks. In terms of important actions this may be setting a DMARD recall window.

Quick and easy tasks might be prompting to calculate a creatinine clearance after receiving a U&E blood sample for a patient taking a DOAC. Alternatively it might be a prompt to save a Q-Risk and semi-automatically send an “offer of statin” letter customised to that patient.

This all ties back up with the dashboard so that you can monitor performance improvements:

Create Automated Letters

These templates also contain links to create automated letters that are based on a patient’s personalised metrics. For example, to facilitate achievement of the IIF metric CVD-06 (Edoxaban switches), users can press a button to check what dose of Edoxaban should be prescribed (this is a safety check and should be used alongside clinical judgment) and then creates a letter that explains why the switch is being done and how to do it. An edoxaban switch can be completed like this within a few mouse clicks for the clinician.
