Target Health Solutions exists to solve a fundamental problem in healthcare.
Across the NHS, high quality care is already being delivered every day. But too often it happens in silos, with teams working in parallel rather than as one system. The result is duplication, gaps in care, and missed opportunities to intervene earlier.
We believe the issue is not a lack of activity, but a lack of coordination.
Our work focuses on helping healthcare teams deliver care as a continuous, coordinated process across general practice, community services, hospitals, and social care. By connecting what already exists, we enable Integrated Neighbourhood Teams to work as one coordinated system around the patient.
This applies across the life course, from working age adults with long term conditions to people living with frailty and complex needs.
Our Story
We began our careers on the front line of hospital medicine, where we saw the same pattern every day. Patients with multiple long term conditions and frailty arriving in crisis, often because earlier opportunities to intervene had been missed.
Care was reactive. Services were fragmented. Teams were working hard, but not always working together.
We retrained as GPs to work closer to where care could make the greatest difference, in the community. There, the opportunity became clear. With the right shared processes and tools, care could be delivered earlier, more proactively, and in a way that maintained independence and reduced avoidable hospital use.
This led to the development of population health approaches, systematic identification of risk, and new ways of working across teams.
How we work
Our approach is built on three core principles:
- Coordination: Care should be delivered as a shared process across Integrated Neighbourhood Teams, not separate activities in different organisations.
- Continuity: Care should build over time, with each interaction adding to a shared understanding of the person, rather than restarting at every contact.
- Proactive care: The focus should shift from reacting to crises to identifying risk early and acting before problems escalate.
We support this through shared digital tools, structured processes, and population health insights that enable teams to act consistently and at scale.
Our Solutions
Our work is not centred on a single product, but a set of tools and approaches that support better system working.
These include:

- Recall systems that ensure patients are reviewed at the right time
- Shared assessments that enable coordinated care across teams
iCGA 3.0 is one example of this approach. It enables Comprehensive Geriatric Assessment to be delivered as a continuous, coordinated process across Integrated Neighbourhood Teams, bringing together contributions from across the workforce into a single shared view.

The NHS does not need more services. It needs the ones it already has to work better together.
Our role is to make that possible, supporting Integrated Neighbourhood Teams to deliver coordinated, proactive care that builds over time, improves outcomes, and helps people live well for longer in their own communities.

Dr James Boorer
GP Partner and Founder of THS
I qualified from Bristol University in 2003 and initially pursued a surgical career, working in major London hospitals and contributing to teaching at Oxford University. I moved to the South West in 2007 for higher surgical training, before changing direction into general practice, a decision I have never regretted.
My work focuses on improving care across the system for working age adults living with long term conditions, particularly heart failure, diabetes, and multimorbidity. I recognise that the NHS already delivers much of this care, but often in fragmented ways, and I am motivated to help bring it together into a more coordinated whole.
This means listening not just to patients, but to system partners across general practice, community services, and secondary care, understanding where care breaks down and where it can be better connected.
Aligned with the values of Target Health Solutions, I focus on bringing these perspectives together, using data, collaboration, and digital tools to connect teams and support a more joined up approach. My aim is to drive measurable improvement and enable more proactive, continuous care for people living with complex long term conditions.

I’m a GP partner at Pathfields Medical Group and Director at Target Health Solutions (THS), with a focus on frailty, population health management, and commissioning.
My work is grounded in a simple reality: the NHS already delivers the components of high quality frailty care, but too often in fragmented silos rather than as a coordinated whole, delivered across neighbourhood teams.
At THS, I use data and digital tools to connect this existing work, enabling teams to function as one system. The aim is clear: better outcomes, fewer hospital admissions, and more people living well at home.
I believe the best care happens when general practice, community services, hospitals, social care, and the voluntary sector work as a single coordinated workforce, supported by shared data and trusted systems.
Alongside my clinical and leadership roles, I’m also a keynote speaker and published author, sharing practical examples of what happens when care is redesigned around continuity and coordination.
